How to make a claim

We know that if you need to make a claim, you want the process to be simple. That's why we've made it possible in just 5 easy steps.

Step one

Get treatment.

Check how much of your annual allowance has already been used.


Step two

Pay the bill.

  • Pay your bill directly with the practitioner.
  • Ensure that you get receipts for any bill you pay.


Step three

Order a claim form.

  • Phone us on 0845 605 0187 if your policy number starts 15 or 0845 600 1561, if your policy number starts 25 or 95.
  • We'll send you a claim form
  • Alternatively, if your policy number starts 15 download a claim form here.
  • Or, if your policy number starts 25 or 95 download a claim form here. 


Step four

Send claim form and receipt to us.

  • Check and sign the claim form.
  • Send the signed form to us by post with the receipt.
  • Keep a copy of anything you send to us for your own records.


Step five

We pay you.
You should receive payment from us within 7 working days.
