How to make a claim
We know that if you need to make a claim, you want the process to be simple. That's why we've made it possible in just 5 easy steps.
Step one
Get treatment.
- Check before you go exactly what you can claim back under the terms and conditions of your plan. You can download your cash plan handbook here.
Check how much of your annual allowance has already been used.
Step two
Pay the bill.
- Pay your bill directly with the practitioner.
- Ensure that you get receipts for any bill you pay.
Step three
Order a claim form.
- Phone us on 0845 605 0187 if your policy number starts 15 or 0845 600 1561, if your policy number starts 25 or 95.
- We'll send you a claim form

Step four
Send claim form and receipt to us.
- Check and sign the claim form.
- Send the signed form to us by post with the receipt.
- Keep a copy of anything you send to us for your own records.
Step five
We pay you.
You should receive payment from us within 7 working days.
